Relinquish

 Completing this form is the first step to beginning the process of relinquishing your dog(s). We understand that parting from your dog(s) is an extremely hard thing to do. We are here to make it easier for both you and the dog(s). Completing this form does not guarantee we will take your dog, but it is our starting point. Please be very honest about all information pertaining to your dog(s). If we accept your dog(s), knowing all about them will help us find them the best home by matching the information you provided. After this form is sent to our intake coordinator, they will reach out to you. At the time of the appointment, you will need to provide proof of vaccinations if any, and spay or neuter paperwork if applicable. There will be a relinquish fee, which will be explained to you when we reach out to schedule a temperament test. Please note: we are not able to take in dogs that have behavioral issues towards other dogs or towards strangers.
